Each parish or organization that has an upcoming Pontifical Mass celebrated by Bishop Bambera is required to complete a Mass form with the details of the liturgy (music selections, Scripture readings, number of altar servers, special rites to be celebrated etc.). This information must be submitted at least two weeks before the parish celebration.  The information can be entered below, or you can download the PDF or Word file of the Pontifical Mass Form and return to Msgr. Dale R. Rupert, Director for Worship.
