Explanation for Criminal History Record Information
and Employment Eligibility  
 

1. Criminal History Record Information

            A.  Pennsylvania State Police

                 Complete the copy of Pennsylvania State Police Request for Criminal
                 History Record Information Form.

            B.  Federal Bureau of Investigation  

                     1.  If you are a non-resident of Pennsylvania, you must complete an FBI
                     Fingerprint Card.

                 2.  You must be fingerprinted by the State Police or Local Police.  

                 3.  Have a $24.00 check payable to the Federal Bureau of Investigation.  

                 4.  The State Police will forward the Fingerprint card and check to the
                      FBI.    

2.  Employment Eligibility Verification Form I-9  

            A.  Applicant completes information contained in Section 1 of the form.  

            B.  Principal completes Section 2.
